About This Job

Jobsplus Permit 175/2017

Are you a Social Worker who is passionate about supporting persons with disability to live more independently? Would you like to help service users prepare for and successfully transition into community-based living?

Aġenzija Sapport invites applications for the position of Social Worker within the Transition Service.

Aġenzija Sapport encourages applications from both newly graduated Social Workers as well as experienced Social Workers.

You will work closely with service users, their support networks and a multidisciplinary team to assess individual needs, coordinate appropriate support and address barriers to successful community living. The service user’s wishes, strengths, preferences and personal goals will remain central throughout the process.

Clarification on the Nature of the Role

Applicants are to note that this call is for the professional role of Social Worker. The role is distinct from that of a Support Worker or Carer. Social Workers are responsible for social work assessment, care planning, case coordination, advocacy, crisis intervention, liaison with families and professionals, and participation in case conferences and reviews.

While Social Workers may work directly with service users and their families, the role does not primarily involve personal care duties or daily hands-on support tasks typically associated with Support Worker or Carer roles.

You will be responsible for:

  • Carrying out comprehensive social work assessments and identifying the support required for a successful transition.
  • Developing person-centred transition plans in collaboration with service users and the multidisciplinary team.
  • Supporting access to housing, social benefits, community resources, education, employment and other relevant services.
  • Providing psychosocial support, advocacy and guidance to service users and their support networks.
  • Identifying and responding to safeguarding concerns, social risks and difficulties affecting the sustainability of community living.
  • Coordinating follow-up, discharge and handover arrangements with referring and receiving services.
  • Maintaining accurate case records, reports and documentation in accordance with professional and confidentiality requirements.

Skills you need:

  • Strong social work assessment, intervention and case-management skills.
  • A sound understanding of person-centred practice and supported decision-making.
  • Strong communication, advocacy and interpersonal skills.
  • The ability to coordinate effectively with families, professionals and other services.
  • Sound professional judgement and safeguarding awareness.
  • Strong organisational, documentation and follow-up skills.
  • The ability to work effectively both independently and within a multidisciplinary team.
